Guiding Dementia is a recently introduced program that builds on our longstanding legacy of quality care at Shanti Hospice. If you are familiar with our hospice services, you will feel right at home with the same quality of care and wonderful care team that you come to expect from us.

Established in 2015, Shanti Hospice is a fully locally owned and operated hospice serving Maricopa County and Pinal County in Arizona.

When the GUIDE Model was introduced, its mission immediately resonated with us. Fast forward to July 2025, and we can proudly say we are one of 8 organizations in the state approved to provide GUIDE Model services!
